Endodontic mishaps or procedural accidents are those unfortunate occurrences that happen during treatment, some owing to inattention to detail, others totally unpredictable. With the evolution of endodontics, improved instruments and equipment¿s and innovative techniques have contributed to a better quality of treatment. The technological advances have changed the way in which endodontics is practiced. These advancements in conjunction with increased dental patient education and awareness have helped to promote the view that the dentition should remain throughout people¿s lives. As the life span of the population increases, the need to maintain a patient¿s dentition for a longer period of time has led to a barrage of advanced procedures, that were non-existent years ago. As a result the need for performing conventional root canal therapy has increased dramatically. This has also lead to an increase in the complexity of the cases treated by both general dentists and endodontics. As a result of this, new problems / procedural errors are being created. Recognition of a mishap is the first step in its management.
