To assess the functioning of IDSP in selected health facilities of West Garo Hills district, Meghalaya. To determine the obstacles and challenges faced by the health workers in reporting of the weekly reports to the PHCs/CHCs in the West Garo Hills district. Qualitative and quantitative study methods were employed in purposively selected health facilities in the district. The participants in the study are district surveillance officer, medical officers, laboratory technicians, health workers and data entry operator and data manager.
