Persistence of malaria poses major threat to health and blocks the path to economic development for individuals, community and nations. Drug resistance is the prime obstacle to management and control of malaria, and is currently progressing at rapid rate leading to shift treatment policy to artemisinin- combination therapy (ACTS) worldwide. This study was carried in Senner area between October 2008-Deceber 2008 with objectives of monitoring the efficacy of Artemisinin-Piperaquine as first line antimalarial treatment, and to validate the role of multiplicity of Plasmodium falciparum infection, parasitaemia and age as predicators for treatment failure outcome. The results showed that Artemisinin-Piperaquine had high cure rate of 97.6% as first line antimalarial treatment in Senner patients. Initial parasitaemia has no significal role in developing resistance to Artemisinin-Piperaquine. Multiclonal plasmodium falciparum infection has significant role in Artemisinin-Piperaquine treatment outcome.
