First, disease like leprosy (lepromatus  granuloma) and diabetes mellitus (systemic metabolic  disorder) lead to bony destruction. It is called  primary or aseptic destruction. Second, bony  destruction also results from neuro-vascular changes  infection and repetitive trauma is called secondary  or septic disorganization.
